Training Class Application

Make sure you fill this out completely. Choose which class you would like to take and make sure you give me the date and time for your preferred class and your second choice. Class must have at least three pets or classes will be canceled or combined with another class. You will be contacted should something like this happen. Please make sure to fill in this information before making your payment, otherwise I will not know what class you would like to attend. When you pay, make sure you add your name, and a contact number to the payment you make to make sure you get proper credit towards your class.

Pawsitive Puppies LLC cannot be held liable for any injury to pet or owner during training. Please keep your dog on a leash at all times in group classes or anywhere around the training area. Never put yourself, the trainer, or your pet in danger.

This is where you can choose how you would prefer to pay. You can choose to mail your payment to me, give it to me personally or use the convenience and security of paying through PayPal. I, personally, have no means of taking credit cards or checks at this time. If you are using one of those payment methods, I would prefer that you use the PayPal option. It keeps your information safe and secure. I have set up the link to PayPal under each service so that you may use PayPal. Dog Training

Group Classes - $80.00

Private class - Package pay 6 weeks in advance $325.00 a $65.00 savings.***mileage will calculated and paid on date of service

Private Session -Single $65.00***mileage will calculated and paid on date of service based on current Government rates.
Pet sitting services are priced below. Remember these are basic prices they do not include mileage.

Pet sitting - 1 Cat $15.00 Mileage will be calculated and paid on date of service based on current Government rates. Pet Sitting - 2 cats/1 outdoor dog - Once daily $20.00 Mileage will be calculated and paid on date of service based on current Government rates. Pet Sitting - 1 dog, two visits daily - $30.00 Mileage will be calculated and paid on date of service based on current Government rates. Pet sitting - 2 dogs, 2 visits a day - $38.00 Mileage will be calculated and paid on date of service based on current Government rates. "Ask The Trainer" Class - YOU MUST PRE-REGISTER FOR THIS CLASS
